Many scientists from all around the world and Lithuania have analyzed the changes of public administration, health care system and management problems. The management of the health care system is important to every part of society as the most sensitive areas of society, such as quality of life, life itself, well-being and social well-being, depend on the efficiency and effectiveness of the system. Although a large part of the state budget funds collected from taxpayers' contributions is allocated to this system, these resources will never be sufficient to meet all public health needs. In this way, the health care system becomes a target and generates a lot of discussion about the quality of management. There are many various negative factors to pay attention to, such as rising healthcare costs, lack of human resources, an aging population, disease migration and resilience, increasing demand for health services, falling responsibility for one’s health, increasing complaints about poor quality and uneven standards of service in the countries. All these require responsible and effective, being able to adapt to the changing environment, healthcare system management. The identification of management problems and causal links should be one of the most important tasks of healthcare system developers in order to structurally analyze and optimize the management of the health care system. The novelty of the research is that the health care system management issues are being analyzed from the position of the authorities involved in the management of the health care system. The scientific problem of the project can be formulated as a question: why is the modern health care system in Lithuania inefficiently managed? The object of the research is the management of modern health care system. The purpose of the research is to conduct a study of the modern health care system management, which will help to identify the problems of the management and their solutions. The main objectives of the project are: to analyze the concept of management, its factors and their application in the management of the health care system, to discuss the changes of the concepts of the health care system and the issues of improving the management; to analyze the management of health care systems in Lithuania and advanced countries by comparing the performance indicators of health care systems and revealing the factors and extent of reform in health care system management in Lithuania, to investigate the opinion of the institutions involved in health care management about the problems of health care system management in Lithuania. Research methods used in the project: analysis of scientific literature, analysis of legal documents, secondary analysis, survey, statistical and comparative analysis. By using the method of scientific literature analysis, the analysis of public health attitudes, the concept of good and efficient management, and the importance of the health care system is done. The analysis of legal documents is used to analyze the legal regulation of health care system management, while using the secondary data analysis method, let identify the dynamics of the performance indicators of health care systems in Lithuania and in the advanced countries as well as the health care system management problems in Lithuania. By using the survey method the opinion of the institutions involved in health care management on the health care system management process and related problems in Lithuania was analyzed. The methods of descriptive statistics methods have been applied to analyze collected data. The research shows that the key issues of a modern health care system management are related to improperly implemented management functions and not ensuring strategic management of the healthcare system at all levels. The Lithuanian health care system faces reforms for a long time now, but Lithuania lags far behind the EU average in the area of health. Constant reforms aimed to make the healthcare system better meet the public health care needs and provide more efficient healthcare services. The study revealed that the Lithuanian health care system has a lot of management problems: inefficiency, low effectiveness, lack of quality and strategic management. One of the main problems identified in the study is the availability of services and the excessive number of beds in hospitals. Lithuania stands out as one of the countries where residents are treated in hospitals significantly and systematically more often than in other EU countries. This leads to high healthcare costs and additional resources to ensure an efficient, high quality and safe healthcare system. The study shows that, in order to ensure an efficient and effective healthcare system, it is important to provide equal treatment for all citizens and to increase the influence of members of the public and stakeholders on health care issues and decision-making, while the health policy goals must reflect in the strategic management of the healthcare system. The project structure consists of three parts. The theoretical aspects of the health care system and its management by revealing the content of public management concepts, principles of good management, and directions of public administration improvement are being analyzed in the first part of the project. This section focuses on the theoretical aspects of the concept of health in order to reveal the change in the perception of health over time and the influence of health determinants on decisions related to healthcare system management. The implementation of health policy and the principles of healthcare system management is analyzed by identifying the health care management problems. The second part consists of the analysis of the Lithuanian health care system development, the issues of management and its reorganization, as well as the practice of advanced countries. The third part presents the analysis of the indicators of the health care system management and the opinion of the institutions involved in health care management on the health care system management process in Lithuania and related issues. This section identifies the key health management issues faced by the authorities involved in the management of the healthcare system. The final part of the project presents the conclusions of the project, which reflect the purpose of the research formulated in the introduction of the project and its complementary tasks. |
